Custom Questions - Intake Form

I submitted an intake form to review while I am working on it and I noticed that custom questions are not appearing for me once I look at the completed form.

    got this one fixed, intake form submissions are now fully preserved

    Hey @Elly Glenn . Looks like the custom questions’ answers somehow got overridden here after you made an update to the form. I’ll look into this one 👀
