Feature Request

Instead of a slider for age and a separate birthdate field, it would be nice if the age automatically updated based on the birthdate field.

    Added a tab selection for age entry to set it based on birthday, or via the slider if birthday is not known.

    This is not available on intake forms yet; a bit more work is needed.
