Allow a separate description field for questions on intake forms.

Ex.

Questions Title: Do you have active cameras in your home?

Questions Details: While your pet care provider is comfortable with cameras in public spaces, locations must be disclosed prior to their visit for safety reasons. Any cameras not disclosed will result in an immediate cancellation of your booking with no refund.
