Intake Forms

Goal: As a provider, I want to create custom forms so I can collect any type of information I need from clients (intake details, agreements, special instructions, documents, etc.) without managing separate tools.

Scope:

• UI for building forms (text, multiple choice, date, file upload, etc).

• Integrate forms into onboarding & online booking workflows, or share standalone.

• Responses saved into client/pet profiles
